Summit Medical Associates earns national recognition

Hermitage, TN The physicians and providers of Summit Medical Associates (SMA), P.C. recently announced that SMA has achieved Level 3 recognition from the National Committee for Quality Assurance (NCQA) for their Patient Centered Medical Home (PCMH) model of care.
According to a news release issued by SMA, Level 3 is the highest level of recognition provided by the NCQA, which is considered by many to be the gold standard for evaluating healthcare quality and value.
The PCMH is a model of primary health care that is relationship-based with an orientation to the whole person. The primary care physician serves as the leader of the multi-faceted healthcare team, coordinating all of the patient's care and ensuring that patients receive the best, most comprehensive care available. When a specialist is required, the medical home physician coordinates the visits and follows the care throughout the duration of the specialist's treatment.
NCQA medical home qualifications are based on thorough and precise criteria for a number of healthcare standards. SMA has received superior scores in all criteria and thus attained the highest level of recognition, including:
